appointment. The Student Service Office is located in 1157F of Ivy
Hall at 3101 S. Creasy Lane, Lafayette. You can also e-mail the
Disability Services
Advisor.
- Here you can discuss accommodations you may need in your
classes at Ivy Tech and the documentation needed to certify any
disability[ies].
- You will also receive an application for admission, transcript
request card[s], and information about different course and degree
programs. You will need to arrange a date for the placement
assessment. If you need accommodations on the assessment
[extended time, enlarged text, etc.], you must provide proper
documentation prior to the test.
- If you have questions about financial aid, stop by the
Financial Aid Office in Ivy Hall Room 1144.
- After assessment, you will make an appointment to meet with
your program advisor to discuss course selection. Before the
semester begins, contact the Disability Services Advisor to
complete ACCOMMODATION NOTIFICATION FORM[S] if you need equipment,
testing accommodations, etc., in the classroom. You are responsible
for delivering this form to instructors each
semester.
- The Learning Resource Center [LRC] in Ivy Hall Room 1157 has
tutoring available in several subject areas. Many resources are
available to all students at no charge. For more information,
contact the LRC staff.
