OBTAINING FEDERAL TAX RETURN INFORMATION

To apply for financial aid, Federal Income Tax information is required. If you cannot locate a photocopy of your Federal Income Tax Form...

  • Contact the Internal Revenue Service [IRS] at [800] 829-1040 and request a summary of your tax return called a "Letter 1722" for a particular tax year; the Letter 1722 is mailed to the tax filer free of charge within 2-3 weeks or,
  • Visit the local IRS office at 620 Morland Drive, Lafayette, located off of Kossuth Street between Farabee Drive and Sagamore Parkway; the office is open 8am - 4:30pm on Mondays only April 16 to December 31, Monday - Friday January 1 to April 15; the office is closed on federal holidays or,
  • Contact your tax service if utilized, such as H.R. Block, Lightning Tax Service, etc.
